Care coordinators review clients' health, personal, and retirement living needs giving the opportunity to assist the clients and/or family with access to the services and support the client may require. The Care Coordinator advocates to assist the client with understanding and managing their own health and wellbeing, referring to social workers, community support systems, health and wellbeing coaches, and other professionals where appropriate.
Care Coordinators work with the client and family to put a plan of care together. Clients benefit from the care coordinator assisting as the elder care locators, with the transition of care monitoring and following up with the client to ensure client needs are being met.
Our goal is not only to assist our clients with transitioning to their next stage in life but also to assist with the plan that focuses on improving and adding stability for independence and dignity regardless of the client's health, social, and living situation.
Independent Living communities are apartment-style living, good for active older adults who are ready to downsize their living style and the responsibilities of house chores. These communities are set for older adults who are able to do daily activities with little to no assistance and living happy lives within their retirement living community
Assisted Livings are residential environments that give older adults personal assistance within a residential setting. These communities offer apartment-style living with assistance for those seniors whose health and well-being may require a higher level of support. Assisted living allows for older adults to live a social lifestyle where they are able to get assistance with active daily living skills, build friendships, and engage in social activities which add to their social needs.
Board and Care Homes are a group home type of residential care facility for the elderly. These homes offer residents room and board with personal care assistance of active daily living skills. Board and cares are usually single-family homes with 6 residents or fewer, some offer shared and provide rooms and bathrooms, and 24-hour personal care attendants.
Memory Care Communities assist residents with personal care, active daily living skills, housekeeping, transportation, meals, and recreational activities. Memory care is a residential care setting for residents seeking long-term care with intensive specialized care for those with memory issues. Memory care may usually have safety measures in place such as secure entrances and exits, Program activities and events.
In-home Care offers non-medical caregivers to provide assistance with active daily living skills within the client's home. This type of caregiving service offers clients professional support allowing their clients to live safely and independently within the environment of their own home. The caregiver may assist with many ADLs such as meal prep, light housekeeping, bathing, dressing, grooming, medication reminders, and transportation.
Independent Living Facilities known as ILF are privately owned and operated facilities offering low-cost shared housing for lower-income adults, adults with disabilities, and adults with mental illness who may benefit from living within a shared environment. ILF residents must be independent of all ADLs with the ability to live a lifestyle without supervision.
